$45. Registrants will receive a copy of Horvitz & Levy's
15th Annual Review - 2005 Edition.

Registration Instructions:
- Click
here or paste the following link
into your browser and go to: http://eventservices.raindance.com
- Select the "View Scheduled Event" icon,
and find this event, HORVITZ & LEVY.
- Click the green register button to the right of the event
name.
- Scroll down to "I need to set up an account".
- Fill out the Account information and create a User ID
and Password.
- Click continue to pay for the seminar
through PayPal.
You will receive an e-mail with your participant user name
and password, as well as an e-mail confirmation of your event
registration (including instructions for attending the scheduled
